FairT Point comnunications IMPORTANT INFORMATION ABOUT MARIANNA AND SCENERY HILL TELEPHONE COMPANY DIBIA FAIRPOINT COMMUNICATIONS TELEPHONE SERVICE AND THE LIFELINE PROGRAM Marianna and Scenery Hill Telephone Company d/bla FairPoint Communications provides a wide variety of products and services including the following basic service offerings in the Marianna, PA (267) and Scenery Hill PA (945) exchanges Single Party Residential Service Single Party Business Service Monthly Rates S19.50 $24.20 One-Time Charges $46.80 S46.80 In addition to the above monthly rates, a Federal Subsciber Line Charge applies to each line as follows: S650 Residential lines and businesses with single lines 9.20 Businesses with more than one line An Access Recovery Charge also applies to each line as follows $2.27 Residential lines (except Lifeline qualified lines) $3.00 Businesses lines All rates indicated are as of September 13, 2017. Taxes, fees and surcharges are not included in the pricing above Taxes, fees, surcharges and rates are subject to change The above rates include the following: Voice grade access to the public switched network Local Usage Access to emergency 911 services Toll Limitation for qualified low income customers FairPoint Communications provides a Lifeline Program discount for certain residential voice or qualifying broadband services for eligible low-income customers. The Lifeline Program is a government benefit program. For qualified customers, federal monthly support of $9.25 is available. If the discount is applied to voice service, free toll blocking may be available to control long-distance usage. Customers using this offering can still use pre-paid caling cards or dial-around services to place long-distance calls from their homes To reoeive the Lifeline Program discount, residence customers must provide a completed application proving eligibility to FairPoint Communications. Documentation proving enrolment in a qualified govemment benefit programor establishing household ncome at or below 135% of the Federal Poverty Guidelines must also be provided. The Lifeline Program benefit is non-transferable and is open only to eligible customers. Only one Lifeline Program discount is available per household on either a wireless or wire-ine service. Consumers who wilfully make false statements in order to obtain the discount can be punished by fine or imprisonment or can be barred from the Lifeline Program. All customers who receive the Lifeline Program discount must recertify eligibility annually. If you receive a recertification request, you must respond as specified or FairPoint Communications is required to remove the Lifeline Program discount from your account.
